Thanks for submitting the rental form. We will be in touch shortly! In the meantime, as you plan on renting space at Mayesh, we need to collect a Certificate of Insurance (COI) from you. Please download the form, have your insurance company or agent fill it out, and upload it to our system using the form provided.
GET AHEAD ON YOUR RENTAL REQUEST
We will need you to sign & fill out the following two documents to complete your rental request.
Once complete, please email them to your sales rep.
If you have not already submitted your COI, please submit it above or send it to your sales rep at least three business days before your rental.
Without your COI, we cannot guarantee your rental dates.
The following forms are optional for any delivery needs you may have.
Please note: Not all Mayesh branches may have the capacity or availability to provide delivery options.
